Here are a few thoughts. I used to have a dodge W200 Which is a much smaller truck but the problem sounds similar. I fought with it for weeks ( I am definately not a mechanic) before I got it right. In the end it turned out to be a cracked intake manifold and a botched up carb. I had bought a rebuilt carb and couldn't get it to hardly run. I had suspected the manifold but couldn't see anything wrong with it. I finally removed the intake, still couldn't see a problem. So took it to be checked by a shop. On the way to the shop I saw it in the right light, a hairline crack. I put in a new intake, and solved all the problems. And that truck was the best gas mileage best power nicest driving truck I ever had. Untill the body rotted off.
