1957 GMC Grinding gears

Help?!
"Buford", our 1957 GMC 350 dumper, is grinding gears in 3rd and 4th. By the time we get him into those gears, we have lost momentum, and have to shift back down. Is this a clutch issue? Are there sincros that could be bad? Also we have a vaccuum assisted hi-lo range shifter in the rear end. Very hard to get it to change if at all. Help? Thanks for your help!
