1995 F-150 engine problem

My brother"s "95 F-150 with 5.0 engine left home this morning running fine. 5 miles later, he said it sounded like the valves were rattling. He pulled over and it died - wouldn"t start back up. I got there and cleaned the cap, hooked up jumper cables, and checked for spark at the plugs. Spark was there, engine would roll over, but it wouldn"t start until I held the accelerator all the way down and cranked on it for 10 or 15 seconds or so. The engine fired up and ran like a new one. He headed home and radio"d me after about 4 miles saying the rattling had started back up. We stopped and it sounded to me like a lifter was tapping. He turned on the a/c and the engine died - and a little blow by came out the dipstick tube. We let it sit a couple minutes and it fired back up - had to hold the pedal down this time as well.

The truck is in really good condition to have 225,000 miles on it - all highway.

What"s causing the "sometimes" tapping? Sticking valve? Time for a new motor?
