There is a pilot on the front of the torque convertor, and that has been known to come off (or part of it), and if it did, that would likely cause your continual problems. It is not as precise of a fit as a pilot bearing for a manual transmission, but it is quite close. Also too, If a manual to automatic transmission swap was done, the old pilot bearing has to be removed from the crank. If the nose of the torque convertor broke (and some did in that era), or was ground down, or something like that, it could cause problems too. There should also be a steel torque ring under the heads of the bolts that hold the flywheel to the crank. Those bolts also have to be torqued to spec, but not overtorqued, as they can distort the flywheel. Andy hits on a couple of other good points, but there may also be an issue with alignment of the engine and transmission, If you installed the torque convertor to the flywheel, and then installed the transmission, this could have been a cause of some problems ( I havve seen this attempted). If there were offset alignment pins that moved out of position (or even just the straight pins removed entirely), this would lead to excessive stress on the flywheel too. It is also possible too that the front pump on the transmission may not be right, or a bearing is failing in the front pump of the transmission.
