Cab over engine question ....

I've often wondered over the past 15 years or so .... why don't you see many cab-over-engine tractor units on the road anymore? You see a few but most of them are getting pretty long in the tooth. Couple of questions, first ... what was the advantage (or disadvantage) of the cab over engine design? Second, are they still being made and in what numbers compared to conventional cabs? And why so few anymore?
